Academic Progression Appeals (pdf)
Students who believe they have met the minimum standards for academic performance in their previous term, but were not deemed eligible to advance by their program, may contest the status of their academic progression.
Advanced Credit Appeal Form (pdf)
Students may use this form to appeal advanced credit decisions (transfer or PLAR credits).
Applicant Name History Form (pdf)
Applicants who have applied to NAIT with a different name than the name listed on their official educational documents or transcripts must submit this form with supporting documentation.
Apprenticeship Disclosure of Personal Information and Authorization (pdf)
Apprenticeship students can authorize NAIT to disclose personal information to their current or prospective employers.
Changes to Student Record Consent Form (pdf)
Allow a third party to make changes to your NAIT student account on your behalf, including changing personal information, changing or cancelling your enrolment. This form needs to be submitted to the Office of the Registrar.
Course Extension Request Form (pdf)
Students may request up to thirty (30) calendar days of extension if they are not able to complete the required coursework within the normal timeframe of the term due to extenuating circumstances that are beyond the student’s control. Refer to section 4.6 in the Grades procedure for more details.
Course Withdrawal Form
Students who have completed more than 12% of their course and no longer wish to continue may request to be withdrawn from the course. Students who have completed less than 12% of the course may withdraw through their MyNAIT Portal.
Compassionate Withdrawal Form (pdf)
When extraordinary circumstances (such as critical medical condition) beyond the student’s control prevents a student from following the regular drop or withdrawal timelines, a student may submit a Compassionate Withdrawal Form to request a withdrawal without academic penalty and a pro-rated refund. Refer to Cancellation and Withdrawal procedure for more details.
Completion Timeline Extension Form (pdf)
Students who want to extend their program completion timeline must complete this form with their program leader.
Deferred Exam Request Form (pdf)
Students may request a deferred exam for exams weighting 20% or higher if they are not able to complete the scheduled exam due to extenuating circumstances beyond the student’s control. Refer to section 4.5 in the Grades procedure for more details.
Deferred Exam Appeal Form (pdf)
Students may use this form to appeal deferred exam request decisions.
Disclosure of Personal Information Consent Form - Office of the Registrar
Students may authorize NAIT to release personal information to a specified third party. This form needs to be submitted to the Office of the Registrar.
Grade Reappraisal (pdf)
Following the final grade being posted, students may request a reassessment of their final grade.
Program Re-admit Form (pdf)
The readmission process is for students who have been withdrawn from their program and wish to resume their studies. Re-admittance is only granted to eligible students who have met the specific requirements of their programs.
Program Transfer Request Form (pdf)
NAIT supports our students in all their educational pursuits, including the opportunity for program transfer.
Program Withdrawal Form
Students who need to withdraw from their credit program on or after the start of their first class must complete a Program Withdrawal form with their Program Leader and submit it to the Office of the Registrar.
Re-open Application Form (pdf)
Students who have had their application cancelled for failing to submit requirements or paying their tuition deposit can re-open their application for the same program and intake. Refer to the form criteria for more details.
Student Name Change Form (pdf)
Students may submit documentation to have a name change processed on their student record.
Student Protected Information Change Request Form (pdf)
Students who need to have sensitive or protected information changed on their student record, such as their Social Insurance Number (SIN) or citizenship status, must complete and submit this form with supporting documentation.
Student Record Reference Request Consent Form (pdf)
Students may submit this form to request permission from their instructor to provide a job reference.
Transfer Credit Application
Students who meet specific requirements may be eligible for transfer credit from another institution. Students can apply for transfer credits in their MyNAIT portal by selecting Open Self Service > Academic Records > Transfer Credit > Apply for Transfer Credits and complete the required fields.
