Incident/Event Reporting - myCority

Incident reporting has moved from the paper-based incident reporting form to a new online reporting system called myCority. Through this system, staff will be able to report injuries or illnesses, security or environmental incidents, property damage, near misses and hazards.
myCority is accessible through smartphone, tablet or desktop. Once you file an incident/event report, your supervisor and Health, Safety and Environment (HSE) will receive a notification email, and an incident number will be generated. HSE may follow up for more details if needed.
Incident Response and Reporting Procedures:
- If the injury, environmental incident, near miss or hazard is an emergency, call 911 immediately, afterwards call Protective Services at 780.471.7477. Notify the supervisor and submit the online incident/event report .
- If the injury is not an emergency (non-urgent/non-life-threatening). Provide first aid using the closest first aid kit or report to Health Services. Notify NPS, if the incident is related to property damage or possible theft. Report it to the supervisor and submit the incident/event report within 24 hours of the incident.
- Near misses and hazards are to be reported through the incident/event report. The Supervisor is required to address or mitigate the hazard if it is within their program/department. If this is a common area, contact HSE@nait.ca.
- Any of the incidents requiring Maintenance and Operations support, the Supervisor is required to complete a Work Request form using the online maintenance work request.
- In the event the incident/event report is not available, a hard copy of the Health and Safety Incident Report form is available, or you can contact HSE@nait.ca for further instructions.
